7.9 Holidays

INSITE has elected to observe the following holidays during the year:

• ! New Year's Day • ! Memorial Day • ! Independence Day • ! Labor Day • ! Thanksgiving Day • ! Christmas Day

If one of the above holidays falls on Saturday, it will normally be observed on Friday. If one of the above holidays falls on Sunday, it will normally be observed on the following Monday.

Full-time regular employees are eligible for holiday pay immediately upon joining the company. Part-time employees will be paid if the holiday falls on a scheduled workday.

Employees who need time off to observe religious practices or holidays not already scheduled by the company should speak with their manager. Depending upon business needs, the employee may be able to us PTO and take time off. INSITE will work to reasonably accommodate an individuals' specific religious observances.

7.10 Unemployment Compensation Insurance Policy

Unemployment compensation insurance is paid for by INSITE and provides temporary income for employees who have lost their job under certain circumstances. Your eligibility for unemployment compensation will, in part, be determined by the reasons for your separation from INSITE .

7.11 COBRA

The Consolidated Omnibus Budget Reconciliation Act (COBRA) provides the opportunity for eligible INSITE employees and their beneficiaries to continue health insurance coverage under the Company health plan when a "qualifying event" could result in the loss of eligibility. Qualifying events include resignation, termination of employment, death of an employee, reduction in hours, a leave of absence, divorce or legal separation, entitlement to Medicare, or where a dependent child no longer meets eligibility requirements. Contact the HR Department to learn more about your COBRA rights.

7.12 Family Medical Leave Act Policy (FMLA)

INSITE provides FMLA leave to eligible employees in compliance with the Family and Medical Leave Act. Employees are eligible for a covered FMLA job-protected leave if they have worked for INSITE for at least 12 months, worked 1250 hours over the previous 12 months, and if the employee works at a company branch or office location that has at least 50 employees within 75 miles. Basic Leave Entitlement • ! Eligible employees may take up to 12 weeks of unpaid, job-protected leave for the following reasons: ! ! For incapacity due to pregnancy, prenatal medical care, or child birth; ! ! To care for the employee’s child after birth, or placement for adoption or foster care; or ! ! To care for the employee’s spouse, son or daughter, or parent, who has a serious health condition; or ! ! To care for the employee’s own serious health condition
